At Lakeland, safety is our number one priority. We take this matter with the utmost seriousness and expect our subcontractors to do the same. There is a zero tolerance policy. If Lakeland feels any party was endangered due to lack of safety protocol; Lakeland reserves the right to remove them from the job indefinitely.

New Subcontractors Must Provide:

•  Complete Lakeland Prequalification Questionnaire
•  Complete Lakeland Safety Questionnaire
•  Provide OSHA forms 300 and 300A
•   EMR Verification (if applicable)
•   W-9 Form (Signed and dated from within the past 12 months)
•   Minority Certification Forms (if applicable)
